How to report concerns, policy violations, safety issues, or harmful use of SafetyMeter.

We want to know when something is wrong. Whether it is a safety concern, a policy violation, harmful content, discrimination, or a technical issue — please report it. We take all reports seriously and commit to responding promptly and confidentially.

1. What This Policy Covers

This Incident Reporting Policy explains how to report concerns about SafetyMeter, Trusted Tech Africa, or the behaviour of users or actors in connection with our platform. It covers:

  • Harmful, misleading, or abusive use of SafetyMeter's tools or outputs
  • Policy violations (Code of Conduct, Acceptable Use, etc.)
  • Discrimination or harassment
  • Problematic or biased AI-generated content produced by our platform
  • Misuse of SafetyMeter's brand or outputs
  • General safety concerns about our platform's operation
  • Technical faults that may cause harm or mislead users

For security vulnerabilities (e.g. software bugs, authentication flaws, data exposure), please use our dedicated Responsible Disclosure Policy instead.

2. Types of Reportable Incidents

Harmful Content or Outputs

If SafetyMeter has generated content that is inaccurate, discriminatory, misleading, or potentially harmful, we want to know. Our AI narrative layer can make errors, and community feedback is essential to identifying and correcting them.

Misuse of Our Platform or Outputs

If you believe someone is using SafetyMeter to generate false safety assessments, to mislead investors or regulators, to justify harmful technology practices, or to circumvent our terms, please report it.

Discrimination or Harassment

If you have experienced or witnessed discrimination or harassment involving SafetyMeter's team, community, or in connection with our platform, please report it in accordance with our Anti-Discrimination & Anti-Harassment Policy.

Policy Violations

If you believe our platform, team, or any user has violated our published policies (Code of Conduct, Terms of Use, Acceptable Use, etc.), please report it with as much detail as possible.

Brand Misuse

If you encounter someone impersonating SafetyMeter or Trusted Tech Africa, or misrepresenting our brand, tools, or outputs, please report it.

General Safety Concerns

If you have a concern about the safety, fairness, or trustworthiness of our platform that does not fit neatly into the categories above, please report it anyway. We would rather receive a report that turns out not to be an issue than miss a genuine concern.

3. How to Report

What to Include

To help us assess and respond to your report effectively, please include as many of the following as possible:

  • A description of the incident or concern and why you believe it is a problem
  • The date and time it occurred (if applicable)
  • A URL, screenshot, or copy of any relevant content
  • Any information about who was involved (if known)
  • The potential impact or harm you are concerned about
  • Whether you would like to be kept informed of our response

You may report anonymously if you prefer. Anonymous reports may limit our ability to follow up with you, but we will still investigate where we have enough information to do so.

4. Our Commitments

When you submit a report, we commit to:

  • Acknowledge your report within 5 business days
  • Assess the report promptly and impartially
  • Investigate where there is sufficient information to do so
  • Keep you informed of progress (if you have provided contact details)
  • Take appropriate action where a violation or safety issue is confirmed
  • Protect your confidentiality — we will not share your identity with any person you have reported unless required to do so by law
  • Never retaliate against anyone who makes a good-faith report

5. Escalation

If you are not satisfied with our response to your report, or if the matter involves potential harm to a significant number of people, you may escalate to:

  • A relevant data protection authority (e.g. ICO in the UK, your national DPA in the EU)
  • A consumer protection agency in your jurisdiction
  • A relevant sector regulator (e.g. a financial, health, or educational authority if the issue relates to those sectors)

6. False Reports

We take all reports seriously, but we also recognise that bad-faith or malicious reports can harm individuals and waste resources. Knowingly submitting a false or misleading report is a violation of our Code of Conduct and may result in termination of access to our platform.
